check your execcgi config in apache, apache error logs, and cgiirc.config to make sure all cgi/executables are configured correctly.
cgiirc isn't updated much anymore, you may want to also look at qwebirc, which is a bit more modern.

after a netsplit, on rejoin it is the hub that sets any bans from the other side of the split. so while the ban may have been originally placed by joeblow on 2012-03-14, afer the split the list of bans might show it was set by the hub on 2012-05-09.

looks like Mail.php is looking for mime.php in a different location (you include 'includes/mime.php', while Mail.php includes '../php/mail/mime.php' )

there is no command to mass +o everyone. you can have scripts to do it, or do it manually.

the easiest first step is to search the ip on google with the word proxy and see if it shows up in any proxy lists. if that fails, you can try `nmap -T5 -PN -p1-65535 <ip>`. once you have a list of open ports, you can check those to see if they are a proxy using curl/wget/fetch for HTTP proxies, or ...

we can't scan all 65535 ports, only a few hundred of the more common ports are scanned upon connection. we do have some proactive scanners that scrape proxy lists from various websites and scan those to be listed in efnetrbl.

automatic bans for "drones" usually mean your connection info matches that of a known drone pattern. try changing your ident or gecos ("real name").
